- Epinephrine
-
Peripheral Vasoconstrictor
Increases Blood Pressure
It is Adreneline
Increases strength of contractility.
Increases myocardial O2 demand. - Vasopressin
-
Reccomend Vasopressin used instead of Epi for VF and pulsless VT, Used only once in a code.
Vasoconstrictor having less cardio side effects than Epi. - Amiodarone
-
Antiarrhythmic
First line in the treatment for VFib, and V-tach.
Useful in the suppression of v-ectopy in cardiac arrest situations by decreasing automaticity.
ADVERSE- hypotension
replaces Lidocane as drug of choice. - Lidocaine
-
Supresses vent arrhythmias by decreasing automaticity
Has local anesthetic properties
Helps duppress vent. ectopy after MI. and my terminate re-entry vent arrhythmias
Still considered Drug of Chioce by many for vent ectopy
SIDE EFFECTS
Muscle twitching
Paresthesias
Changes in LOC
Focal and grand mal seizures
need to decrease dose by 50% in elderly or those w/ liver disease. - Procainamide
- Supresses Vent. Ectopy and may work when Amioderone and Lidocaine fail, Takes a long time to give. Not drug of choice due to this.
- Bretylium
- Anti-arrhythmic but no longer used d/t nation wide shortage.
- Atropine
- Parasympatholytic agent, Enhances Sinus Node automaticity and AV conduction, used in symptomatic brady, first degree AVB, Type I AVB, Type II AVB, Asystoli, and PEA
- Adenosine
-
Slows conduction through the AV node used in PSVT, and WPW.
!!!SLOWS HEART DOWN!!! - Calcium Channel Blockers
-
Decreases AV node conduction
Ex. Verapamil.
Used for SVT, Afib, and AF.
!!!SLOWS HEART DOWN!!! - Magnesium
- Treats Torsades and Lowers complications with AMI
- Morphine
-
Narcotic analgesic
used for ischemic chest pain, and in acute pulmonary edema to decrease preload - Calcium Chloride
-
Increases force of myocardial contraction
used in hyperkalemia, hypocalcemia, and w/ calcium channel blocker toxicity.
use w/ caution w/ digitalized patients, as it precipitates dig-tox - Norepinephrine (Levophed)
-
Increases contractility, and VASOCONSTRICTS.
Increases SVR and may decrease cardiac output and incrases myocardial O2. - Dopamine
-
used with hypotension, and bradycardia, increases renal and mesenteric vasodilation at lower doses
20 mcg/kg/min is max dose
5-20 is used for hypotension
1-2 is used for renal vasodilation. - Dobutamine (Dobutrex)
-
Increases cardiac index
treat patients with pulmonary congestion and low cardiac output, and in hypotensive patients - Isoproteronol (Isuprel)
- used for bradycardia, used instead of atropene in transplanted or denervated hearts
- Amrinone (Inocor)
-
Decreases preload and afterload, Used for short term mgmt of CHF and depressed myo contractility who respond poorly to diuretics, Dig, and vasodilators, !!!
used to improve CO on CABG patients!!! - Sodium Nitroprusside (Nipride)
- Potent peripheral vasodilator. Used in emergency treatrmetn of hypertension and Heart Failure.
- Nitroglycerine (Nitrol)
- Realxes vascular smooth muscle. Drug of Choice w/ angina.
